Request Personal Reports/Images

You may obtain your personal imaging reports and/or images the following ways:

Open, fill out and submit the secure Release of Medical Records form

Pick up your records from one of our three offices
Call Medical Records directly at 913-754-4707. Leave the following information:

1. Name
2. Date of Birth
3. Medical Records Needed
4. When you will pick up the records
5. Which site you will pick up from (Overland Park, Lee’s Summit, Liberty)

Patient Portal
The patient portal is an online protected site that gives the patient access to their medical reports 72 hours after the exam was read by the radiologist. If you aren’t sure if you have access or would like to sign up call us at 913-469-8998.

Allow one business day for all requests.
You will be required to show a picture id when picking up your records and may be asked to verify other information.

Open, fill out and submit the secure Release of Medical Records form

Fax a request on your letterhead
to 913-469-5695 which includes the patient name, date of birth, records needed, your phone and fax number.

Third Party Medical Records Request

If you are a third party
such as an insurance company, workers’ compensation, disability agency, attorney, etc., open, fill out and submit the secure Release of Medical Records form

Billing questions
